Chatham Oaks provides two main services - Residential Care Facility (RCF) and Home-Based Habilitation services. The Residential Care Facility provides services 24 / 7 for adults with a mental illness. The focus of services is to provide support for the individual to increase skills for independent living. Services include nursing, recreation and skills training. Each resident is assigned a social worker. Home-Based Habilitation is a Medicaid funded service. Services are provided to individuals with a mental illness who need support while living independently in their own home or apartment. Services may include assistance with grocery shopping, budgeting/money management, medication management and problem solving. Chatham Oaks also provides 24 hour supervised services in three homes in the community for 4 or 5 individuals in each home.

Individuals receiving services must be 18 years of age or older and diagnosed with a chronic mental illness. The individual must also be approved for funding for the appropriate services.
Residential Care Facility services are funded by the region of the county of legal settlement or private pay. Service rates are negotiated annually with the ECR Region. Home-Based Habilitation is a Medicaid funded service. Individuals must be Medicaid eligible and first be approved by the regional Integrated Health Home and managed care entity.
